What does the diagnostic process actually involve?
Your Autonomia Advisor will work with you to explore the seven key aspects of home care. The goal is to create a comprehensive overview of the caregiver-care recipient relationship. You will receive a structured report within 48 hours and, depending on the package you choose, a customized support plan.
Can I switch from one plan to another while I'm enrolled?
Yes. If your circumstances change, you can upgrade your plan. Your Advisor, who is already familiar with your situation, will adjust the action plan without you having to explain everything all over again. Consistent support is one of our commitments.
Can Autonomia step in if my loved one doesn't want help?
This is one of the most common situations. Your advisor has experience dealing with these situations. They will guide you on the best approach to take and the steps to follow. Sometimes, bringing in an outside expert can help resolve issues that cannot be resolved within the family.
How soon can we start the coaching program?
An advisor will call you back within 48 hours of your request. For urgent situations (imminent discharge from the hospital, loss of home care services), we will prioritize your request.
What is the process like for a hospital discharge supported by Autonomia?
When a loved one needs to return home within 48 hours, your Autonomia Advisor immediately activates the network: assessing urgent needs (medical bed, home nurse, care assistant, meal delivery), coordinating with the hospital and the primary care physician, and arranging for a local service provider through the OUICARE or Silver Alliance network. Your loved one returns home safely, and you remain the son or daughter—not the caregiver.
What happens once the Support Program has been submitted?
The Customized Support Program is a living document. Depending on the package you choose, your Advisor will oversee the implementation of priority actions: contacting service providers, securing financial assistance, and coordinating stakeholders.
